Pygeum (Pygeum africanum) is one of the more overlooked botanicals in the modern prostate-support landscape, despite having a substantial European clinical research base going back to the 1960s. Originally extracted from an African evergreen and developed primarily by French and Italian researchers, Pygeum has been used clinically for benign prostatic hyperplasia in Europe for over 50 years.

The mechanism

Pygeum bark extract contains pentacyclic triterpenes (ursolic, oleanolic acids), phytosterols (beta-sitosterol, beta-sitostenone), and ferulic esters. These compounds appear to work through several pathways:

  • Anti-inflammatory effects on prostate tissue specifically.
  • Modulation of bladder receptor sensitivity, reducing urgency.
  • Possibly modest 5-alpha-reductase inhibition, contributing to DHT-related effects.
  • Improved bladder emptying through smooth-muscle effects.

The trial picture

A Cochrane review of 18 randomized controlled trials of Pygeum found significant improvements over placebo in:

  • Overall urinary symptom scores
  • Nocturia frequency
  • Peak urine flow rate
  • Residual urinary volume after voiding

Effect sizes are modest — comparable to Saw Palmetto in most trials — but consistent. The combination of Pygeum and Saw Palmetto outperforms either alone in some comparisons.

Standardisation

Side effects
